In the “Select request type” field select “Remove me from Ariat Insider Program”
You will then need to fill out all required fields
In the “Request Details” field type “Remove me from Ariat Insider Program”
Complete the reCAPTCHA at the end of the form
Click “Submit” and we will then process your request to be removed from the Ariat Insider membership program

Please note that requests to opt out of the Ariat Insider membership program may take up to 30 days to become effective.
